首先请您为大家介绍一下患者情况及心耳特点。
该患者CV评分5分，HAS-BLED评分4分。
术前造影，心耳为敞口反鸡翅型；心耳开口低于左下肺静脉，和房间隔下缘齐平。
对于低位反鸡翅心耳，好的穿刺位置是手术成功的关键，所以我们在ICE引导下行房间隔肌部穿刺。
确实，在可视条件下穿刺可以达到事半功倍的效果。
术中封堵器快展开时，适当往前推送，最大利用深度，避免封堵器被心耳挤压出来，符合PASS原则予以释放；术后一周心超检查无心包，术后2个月CTA随访无造影剂进心耳内，心耳已内皮化完全，停抗凝，改为阿司匹林+波立维双抗。
